DTaP (Diptheria, Tetanus and Pertusis/Whooping Cough)
- Required for entry into childcare or school
- Only used for children ages 2 months through 6 years of age
- Free for children through age 6 who are VFC eligible
- Cost is $32.00 for those who are not VFC eligible
DTaP, Hep B and polio combination vaccine (Pediarix)
- Combination vaccine may be used to meet individual vaccine requirements
- Can be used instead of single vaccines for age-appropriate children
- Free for children age 2-6 years who are VFC eligible*
- Cost is $91.00 for those who are not VFC eligible
DTaP, Polio and Hib combination vaccine (Pentacel)
- Combination vaccine that may be used to meet individual requirements
- Free for children who are 2 months through 4 years of age and VFC eligible
- Cost is $95.00 for those who are not VFC eligible
EIPV (Polio)
- Required for entry into childcare or school until 18 years of age
- Free through age 17 for children who are VFC eligible
- Cost is $31.00 for those who are not VFC eligible
Haemophilus Influenza Vaccine (Hib) Note: this is not regular flu vaccine
- Required for entry into childcare or school
- Not given after 4 years of age except for certain high risk children ages 5-18
- Free through age 4 and high-risk children ages 5-18 who are VFC eligible
- Cost is $26.00 for those who are not VFC eligible
Hepatitis A
- Free for children ages 12 months through 18 years who are VFC-eligible*
- $25 for ages 1-18 who are not VFC eligible*
- $74 for ages 19 and up
- 2 doses required
Hepatitis B
- Required for entry into childcare or school
- Free for the following:
- Children age 0-18 years who are VFC eligible
- Household contacts to carriers
- Refugees from endemic countries
- Sexual or needle sharing contacts to acute or chronically infected persons
- Cost is $58 per dose for persons age 20 and older, including those needing Hepatitis B vaccine for employment (3 doses needed)
- Cost is $30.00 for those 0-19 years who are not VFC eligible
Human Papilloma Virus (HPV – Gardasil)
- Free for females and males 9-18 years who are VFC eligible*
- $141 for others
- Recomended for males and females 9-26 years of age (3 doses series)
Influenza (Flu) (injectable)

- Required for entry into some colleges(only for persons ages 2-55)

Pneumococcal Conjugate Vaccine (PCV 13)
- Not required by law
- No cost children under 5 years who are VFC eligible*, uninsured children, underinsured children whose insurance does not cover vaccine cost and for children 6-7 years of age with certain medical conditions
- $116 for those not eligible for free vaccine - may have to order
Pneumococcal Polysaccharide 23-valent (PPV23) (Pneumonia vaccine)
- Free for high risk children ages 2-18 who are VFC eligible
- $45 for non-high risk people.

High risk include those with functional or anatomical asplenia, immuno-compromised, bone marrow transplant or cochlear implant recipients, Alaskan natives or American Indians.

One dose recommended for anyone age 65 and older; Medicare B will usually cover.
Rotavirus (only for children ages 6-32 weeks of age)
- Free for children who are VFC eligible and who are high risk
- Cannot begin series after 12 weeks of age and cannot be given after 32 weeks of age
- $99 for others; for-pay vaccine not always in stock; will order as needed
Tetanus/Diphtheria (Td)
- Required for school attendance if older than 6 years of age and not fully immunized with DTaP previously and not yet eligible to receive Tdap (see below)
- No cost
- Used in age 7 and up; Booster doses recommended for all persons every 10 years (also, see Tdap)
Tdap (Tetanus, diphtheria & pertussis/whooping cough)
- Required for students entering 6th grade in a public school on or after August 1, 2008 if 5 or more years have elapsed since last dose of Td; and for students not in public schools who are 12 years of age on or after August 1, 2008 if 5 or more years have elapsed since the last dose of Td
- One dose is recommended for anyone ages 10-64 to replace one dose of Td
- Free for 10-18 years of age who are VFC eligible
- Free for NC college entrants who have never had Tdap
- Free if needed for wound management
- Cost is $45.00 for others
Twinrix (Hepatitis A/Hepatitis B Combination)

Free for ages 18 and up who meet high risk criteria (3 doses)
(Free Twinrix vaccine cannot be used for the accelerated, 4 dose series or for persons with a documented history of a completed hepatitis A or B series)
- High risk persons are:
- men who have sex with men
- IV drug users
- Persons with multiple sex partners
- Persons who have been incarcerated
- Persons who are Hepatitis C or HIV positive
- Persons seeking treatment for an STD
- Persons with chronic liver disease
- persons receiving family planning services who are not in a mutually monogamous relationship
- Cost is $112.00 for others
Varicella (Chickenpox)
