Child Service Coordination (CSC)

Child Service Coordination is an interagency program available to any child birth to age 5 regardless of financial status who is medically or socially at risk for developmental delays or who has been identified with developmental concerns.  Reasons for referral and enrollment in the Child Service Coordination Program may include any of the following:

  • Parenting questions or concerns
  • Medical problems or concerns
  • Developmental risks or delays
  • Social support needs (such as housing, finance, etc.)

Any person can make a  referral to the CSC program by contacting the Health Department.  Nurses and Social Workers provide these services at no charge to the family.

Services include:

  • Home or daycare supportive visits
  • Review and assessment of family's and child's strengths and needs
  • Referrals to appropriate community resources
  • Parent education
  • Developmental screenings and education
  • Communication with child's doctor(s)
  • Coordination with other agencies involved with family
  • Families in Focus intensive home visiting

Child Service Coordinators can be helpful to families concerned about the following:

  • parenting issues
  • monitoring of growth
  • development and safety
  • accessing child care
  • transportation
  • specialty services needed by the child/family.

Children are seen  primarily in their homes as often as indicated but not less than every three months.  Child Service Coordinators collaborate closely with medical providers to ensure that children are receiving appropriate care and services necessary to promote their health and development.
